Form Instructions
Built-in Directions: To help you use the interactive
forms, most of them include simple directions at the top of the first page
of the form.
Forms Requiring Signatures:
Because many City forms require a signature, which at
this time cannot be
achieved online, most interactive forms must be printed out to your
desktop printer after all pertinent information has been filled out
online. Once printed out, you can sign the form, then mail or
hand-deliver it to the appropriate City office.
To Find Out More About a Particular Subject: If you are
not sure what a particular form is used for, or you have questions
on additional documentation or requirements that may be referenced in a
form, you can search the appropriate department web pages for more
information. The home page for each department is listed as a
header on the Online
Forms web page,
and serves as a link to that department's portion of the website. For example, clicking on the "Grace A.
Dow Memorial Library" header will take you to the Library's
home page; further searching on that home page will reveal a link to
more details about the "Ask The Librarian" online form.
In addition to being posted on the Online Forms page, forms are also located on web pages for the
departments from which they originate.
As more interactive and/or printable forms become available, they will be added to
this website.
